Creating an Unsuccessful Attempt form

This functionality allows users from the Testing Authority or Sample Collection Authority to create Unsuccessful Attempt forms for athletes within a Testing Order.

  1. Open the Testing Order
  2. Within section 2, select an athlete
  3. Click the Other Actions Drop-down and select Add Unsuccessful Attempt
  4. Complete the form which includes the same fields as the WADA template
  5. Save.

Note: the status of a Test is automatically set to "Closed" when three Unsuccessful Attempt forms are completed, Anti-Doping Organizations can, however, manually change its status from the Testing Order.
