Unsuccessful Attempts can only be created for athletes that have whereabouts in ADAMS:
- Within a Testing Order, select an athlete with whereabouts information
- At the bottom on the “Find” tab, click ADD next to the "Unsuccessful Attempts" section
- Complete all mandatory fields
- Click Submit
